Unless you are ambitious enough to do this yourself, try American Stitches.
Dupler or Eng should have their details.

Last quote that I heard from them, it was around ~$52 - $53 to recover the knob. We still have not been able to expand to other colors yet, so your only options right now, are Black or Maroon. They can't do Blue or Terra Cotta yet (they need some good color samples first).
Otherwise, I don't know of anyone w/ a pattern for the leather. I have not intentionally removed the leather from a knob, either the leather is still good & I leave it alone, or the leather is gone already.
Plus don't forget... Underneath the leather is a layer of foam padding... Which American Stitches ALSO replaces & installs.
Item last... You also have to find a way to carefully remove the shift diagram on the top w/o cracking/breaking it. So that the leather gets tucked inside where the diagram circle meets the knob body. AS has a way of getting it off, looks like they use a small punch to free it.

I've got the old crap off and the cap off, i've also got the leather and padding that is close to the factory stuff. I was hoping someone had a pattern to so i coud slice and dice the leather and recover it myself.
